The admission is made directly with the discharge of the National Hospital or private nursing clinic or after recent hospitalization up to 15 days, and on condition that is required immediate rehabilitation. The ambulance of the Center receives the patient and ensures the safe transport to and from the Center.

Documents for  participation in Rehabilitation Program:

1) Medical Report from Director of clinic, stamped by the administrative department for the signature of the director and attending doctor. The report will  describe in detail: a) the nature of the disease, b) the severity of the disease or injury c) effects and their sequences, s) type of recovery and treatment, e) the probable duration of the rehabilitation in accordance with the indicated time EOPYY, f) the expected result,
2) Discharge paper of the hospital with the indicated disease code  which requires admission in the Rehabilitation Center,
3) Sheet patient’s pre evaluation
4) Medical booklet stamped.
5) ID, photocopying,
6) Conclusion CT or MRI in the case of  stroke,
7) Laboratory tests (optional), and medication that continues after hospital discharge.
